About This Portal
Rural Health Transformation Program — North Carolina
This portal brings together three complementary views of rural health capacity in North Carolina: where health needs are greatest (Population Health), what organizational infrastructure exists to meet those needs (County Asset Inventory), and who is delivering care on the ground (Provider Workforce Inventory).
Each dashboard is county-level and cross-referenced against the state's six Managed Care Regions and Urban/Rural designations, so gaps between health need, facility access, and workforce supply can be identified side by side.
